Immune responses following G-CSF mobilized allogeneic blood stem cell transplantation

Rationale: Earlier research from this laboratory demonstrated a significant decrease in immune functions of mononuclear cells (MNC) from 104 G-CSF mobilized normal human donor blood stem cell harvests compared to steady state non-mobilized stem cell harvests. The clinical significance of these findings is not currently well understood. The studies on pattern of immune reconstitution in transplanted patients might be useful information to understand the biology/behavior of the transplanted cells in vivo. Objective: To evaluate the immune functions including natural killer (NK), lymphokine activated killer (LAK) cell mediated cytotoxicity, T and B lymphocyte response to specific mitogens and immune phenotypes of blood MNC from 104 patients transplanted with GCSF mobilized blood cells. Methods: In vitro immune responses were studied before starting the preparative regimen 28, 100, 200 and 365 days after transplantation. The results were compared to MNC from non-mobilized leukopheresis products and pretransplant baseline levels. However, both T and B cell functions as measured by mitogen response do not come to baseline level till day 365 post transplant and did not come back to normal level during the study period. The decreased post immune functions might play a role in less than expected incidences of acute graft versus host disease observed in these patients following G-CSF mobilized stem cell transplantation.
