Patient-centered medical home

Abstract

While the most commonly used term is “patient-centered medical home,” others such as “primary care medical home,” “personal medical home,” “advanced medical home,” and “healthcare home” also exist. Despite variances among these concepts, all share similar core values and principles. Except for direct quotes that refer to a specific medical home, the term “medical home” will be used for this chapter in an effort to be more inclusive.
